The Data Breaches is a Story Mode achievement that Rockstar has not attached a public requirement to, so its details stay unknown until you unlock it. The 16.5% clear rate keeps it in rare territory, close to a handful of other hidden Story Mode achievements around the same difficulty. With no official requirement text to go on, there is not much to add beyond what the percentage implies: a meaningful slice of players get there, but well under a fifth of everyone on Steam. That puts it above the game's most obscure hidden goals but still short of anything close to guaranteed. Guessing at a specific cause tends to waste more time than it saves. Playing normally and letting the notification appear works just as well as any theory.
This is a hidden achievement. Play through the story and it will unlock at the right moment.
Achievements unlock on Steam, PlayStation (as trophies) and Xbox alike. Cheats disable them for the session, so play clean when you are chasing one.
